(Major Placement Contract)
THIS AGREEMENT is between ______________________________________ (the “Student”), ________________ (the “Sports Body”) and Canadian Therapeutic College Corp. (the “College”).
BACKGROUND:
1. The Student is enrolled as a student in the sports injury therapy program at the College (the “Program”).
2. As part of the Program, the Student will complete a field placement with the Sports Body and may provide massage therapy, sports injury therapy and other services (the “Services”) to athletes and other individuals connected with the Sports Body (the “Athletes”).
3. The Student may not be a registered massage therapist and may have no professional or other qualifications to provide the Services to Athletes.
4. The Services provided by the Student will be unsupervised and, in particular, will not be supervised by the College or by a registered massage therapist.
FOR GOOD AND VALUABLE CONSIDERATION (the receipt and sufficiency of which are acknowledged) the Student, the Sports Body and the College agree as follows:
1. The Student, the Sports Body and the College acknowledge and agree that the statements contained above under the heading “Background” are true and correct.
2. The Student agrees not to provide any Services except for Services which they have been trained to provide and are fully comfortable in providing to Athletes. The Student has been advised and has agrees not to act above or beyond their skill or knowledge level.
3. The Student agrees not to provide any Services to an Athlete who has not provided their informed voluntary consent, except in the case of an emergency where it is not possible to obtain prior consent. The Sports Body and the Student will hold discussions with the Athletes for the purpose of explaining the following:
· the field placement and the Student’s role with the Sports Body,
· the Student’s status as a student without professional qualification,
· the fact that the provision of the Services by the Student will not be supervised by a registered massage therapist or other professional
· the nature and purpose of the Services,

· the expected benefits of the Services
· the material risks and any potential side effects of the Services (including risks of minor injury which may occur frequently and risks of serious injury or death, however remote),
The Student will hold separate discussions with each individual Athlete concerning their own specific case and any treatment suggested for them. The Student will explain all of the points described above and will also explain any special factors particular to the Athlete. The Student will explain any proposed treatment and will explain alternative courses of action and the likely consequences of not having the treatment. The Student will answer any questions from the Athlete concerning all relevant matters. The Student will obtain a written consent to treatment from each Athlete before providing any Services to the Athlete, except in the case of emergency where prior written consent can not be obtained.. The Student will keep a detailed written record of all discussions relating to these matters.
4. The Student understands and agrees that, in the event of emergency, they will relinquish care of the Athlete to emergency services personnel (such as an ambulance attendant or physician) once an emergency service has commenced.
5. The College cannot and does not warrant the skills or knowledge of the Student or the quality of the Services provided by the Student to Athletes.
6. The Sports Body accepts the Student as a team/facility member of the Sports Body and shall ensure that the Student is included in any insurance or medical insurance plans available to the Sports Body or its members. For example, this insurance will cover any loss, damage or injury (including death) incurred or suffered by the Student while participating in any events relating to the Sports Body.
7. The Student agrees to use his or her best efforts to attend pre-arranged events of the Sports Body during the field placement.
8. The Student releases the College from all claims or demands arising from or relating to the field placement including, without limitation, any claims or demands relating to any damages, costs, injury (or death) suffered by the Student in connection with the field placement.
9. The Sports Body shall pay the cost of all supplies or consumables required by the Student to provide the Services during the field placement and shall reimburse the Student or the College for all such expenses. The Sports Body shall also reimburse the Student for any approved out-of-pocket expenses incurred by the Student.

The Student shall abide by the Field Placement Guidelines as set out by the
College from time to time.

11. The Student and the Sports Body acknowledge and agree that the Student may obtain educational benefit by participating in the field placement and implementing the skills and knowledge developed under the Program. The Sports Body and its coaching staff and management agree to use their best efforts to support the Student and his or her decisions and to ensure that the field placement is an effective educational opportunity for the Student while providing a benefit to the Sports Body and its Athlete.
12. The Sports Body may terminate the field placement at any time by written notice to the Student and the College.
13. This agreement shall be binding on the College, the Sports Body and the Student and their respective legal representatives, heirs, successors and assigns.
